North Peel FHT Quality Improvement Plan (QIP) - 2019/2020


Every year, we get the opportunity to develop an improvement plan that continues our work of providing quality care to the patients of North Peel Family Health Team.

This year, our plan focuses on what matters to you and your family.

We are aiming to provide our patients with the right care at the right place and time.

Here are our improvement goals for 2019/2020:

Continuity of Care:

We continue to work with our clinics and surrounding hospitals to make sure that you have a connection back to your family doctor’s office within 7 days after leaving the hospital.

Involving and Listening to our Patients:

We continue to offer patients opportunities to provide feedback about their experiences and input into how to improve care. Engaging patients to improve care is a priority for us and with our patient experience survey, we will continue to listen to you as well as keep you updated on the results and actions taken. Access to Care:

We think that all NPFHT patients, and their families, should know when and where care is available specific to the time and day of the week and what issue needs attention. We continue to support this initiative to having on call and after-hours appointments available, as well as surveying your satisfaction, all aiming to provide you and your family with timely access to your care team.

Palliative Approach to Care:

Digital health tools are constantly expanding the types of supports that can be offered to your and your health team. Investigating these tools, as well as tailoring them to fit the needs of our patients and practices, we hope to offer support much earlier for people with life threatening, progressive illnesses. This early planning allows people to get the right care and team in place.

Effective Opioid Prescribing and Management:

Digital health tools are constantly expanding the types of support that can be offered to you and your health team. Investigating these tools, as well as tailoring them to fit the needs of our patients and practices, we hope to better monitor and identify opioid prescription trends, while investigating and implementing best practice guidelines for prescribing and supporting our patients on opioids.
