Ontario Health Links

What is a Health Link?
It has been determined that five per cent of patients account for two-thirds of total health care costs. These are most often patients with multiple, complex conditions. Health Links is a new Ontario Ministry of Health and Long-Term care initiative which provides coordinated, efficient and effective care to patients with complex needs. Within each Health Link, health care providers are brought together to ensure that people are at the center of their care and that family doctors have the ability to connect patients with the care they need.

When the hospital, the family doctor, the long-term care home, community organizations and others work as a team, the patient receives better, more coordinated care. Providers will design a care plan for each patient and work together with patients and their families to ensure they have:

While there are several Health Links which support care in the Halton Peel region, the Brampton Health Link
provides care coordination for the majority of eligible NPFHT patients. The co-leads, Jane Delacey, Brampton
Civic Hospital and Dr. S. Goel, Wise-Elephant FHT administer the Brampton Health Link initiative on behalf of
relevant community partners, e.g. William Osler Health Centre, North Peel Family Health Team, Community Care
Access Centre, etc.
